Under the General Data Protection Regulations (GDPR), which come into force on 25thMay 2018, I am required to give you the following information about your data, what is held, how it is stored and how it is used.

I need to collect personal information about you in order to provide safe and effective treatment.  This information includes your name, address, date of birth, medical history, medication etc.  You do have the legal right to refuse to supply this information, but I would then be unable to provide treatment.

The information is held on paper in note form and also in printed form, from any health form that you may complete on my website.  Your notes are held in a secure filing cabinet, which only I have access to. Your records will be kept for 7 years after your last treatment.  Records are then securely shredded and disposed of.  I also store some electronic data about you, which includes your name, e-mail address and mobile phone number.  This is securely stored on my online booking system, for rebooking and also tax purposes.  If you do not wish this information to be stored, then you can request that it be deleted from the system.

I use a card provider called iZettle. This is so that you have the option of paying by card, however, no credit or debit card information is stored online.

I will never share your data with anyone without your written consent.  You have the right to see what personal data I hold on you, following a written request and also to request that I correct any factual errors.  Any breaches of data security that may affect you will be advised to you within 72 hours.
